If you wish to report an absence,  a late arrival or a medical condition, please email me on jgordon@st-ann-school.org or telephone me directly on 609-883-4336.   Please speak clearly and slowly on any voicemails and leave the name of your child and his/her grade (e.g. 1B  or Mrs Lestician) and the reason.

THINGS I NEED FROM YOU:

If your child has ASTHMA – I need an Asthma Treatment Plan and Medicine order signed by your doctor,  a supply of any medication to be used during school hours (Inhalers, nebulizer treatment with mask and tubing) and a current copy of your child’s physical.

If your child has SEVERE ALLERGIES – I need an Allergy Action Plan signed by your doctor,  a supply of any medication to be used in an emergency (2x Epi-Pen, or 2x Auvi-Q ,  Benadryl) and a current copy of your child’s physical.

If your child has SEIZURES – I need a Seizure Action Plan/Medication Order signed by your doctor,  a supply of any medication to be used in an emergency and a current copy of your child’s physical.

If your child needs MEDICATION DURING SCHOOL HOURS – I need a Medicine order signed by your doctor,  a supply of any medication to be given  (must be in original labelled bottle/box)and a current copy of your child’s physical.

Please be aware – The Nurse does not have Stock Medications – anything your child may require must be supplied by you, signed by a parent and counter-signed by your doctor.  We have a special form for over-the-counter Non-prescription items such as tylenol, eyedrops, tums etc.  This can be obtained from the Nurse or front office.

PLEASE  send in a copy of your child’s VACCINATION RECORD when he/she receives any new ones during the school year.

FLU SHOTS – Preschoolers under 5 years of age are required in NJ to receive Flu shots on or before 31st December of the current year and may NOT return to school until the school has received documentation of the vaccination.
